SUMMARYProvides information and referral, agency linkage, advocacy, home visits, transportation to and from appointments and recovery support for clients receiving medication assisted treatment services. Provides crisis intervention and emergency services, as well as utilizes center-wide and community resources as needed for client benefit. Embraces the key values of case management: empowerment, normalization, rehabilitation, and continuity of care. If applicable, completes BHSN (Behavioral Health Safety Net) applications and Cover Rx applications and renewals with clients to ensure consistent coverage. Maintains a caseload of clients who are seen at least once per month and liaisons on behalf of clients with treatment team and community resources to ensure client's daily needs and recovery goals are met. Maintains compliance with clinical standards and productivity set by the program. This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment. Completes other tasks as assigned by the Program Coordinator.
Education/Knowledge: Must have at least a Bachelor's degree in a health-related field of counseling, psychology, social work, sociology, or a behavioral science field with course work or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and intellectual disability. Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, or education, must have fifteen college-level semester hours of coursework in behavioral health and at least one year of work experience in the behavioral health setting.
Experience: One year experience in the social work field or a behavioral health setting preferred. Certification as a Peer Recovery Specialist preferred, but not required.
Abilities: Exposure to biological hazards. Hearing of normal and soft tones. Close eye work. Valid driver's license with an "F" endorsement. Lifting up to 50 lbs. Pushing/pulling up to 150 lbs. frequent sitting, standing, walking, bending, stooping, and reaching. This position requires certification in and adequate implementation of verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, and going down on knees, running, and walking.
