JOB PURPOSE/SUMMARY
Summary of role of team:
- A Centerpointe LPN is responsible for the assessment and management of withdrawal sign/symptoms of clients presenting to the unit.
- The Centerpointe LPN will work under the direct supervision of the Team Leader.
SUMMARY OF POSITION:
- The Centerpointe LPN will have strong communication skills, good time management and organization skill.
- Has excellent teamwork and collaboration skills.
- Will provide direct care to clients in the Residential/Detox setting.
- Will possess a working knowledge of basic nursing techniques.
- Will daily develop a to-do-list for each client and follow it considering client's needs and acuity of need.
- Will document three times per shift and PRN.
TYPICAL WORKING CONDITIONS/ENVIRONMENT
- This position is based within Centerpointe Detox and Residential services.
- Hours of operation on 24/7, 365 days per year.
- This position requires working different shifts as needed.
1. Quality Care
- Works in a multi-disciplinary team approach to meet medical needs of individuals suffering from substance use disorders/co-occurring disorders.
- Will complete initial, comprehensive assessments of clients upon admission, which includes appropriate documentation and integration of assessment findings of vitals, CIWA/COWS, TB Testing, Urine Testing, etc.
- With medical guidance, will initiate appropriate medical protocols for clients.
- Will administer medications with 100 percent accuracy.
- Will provide indirect and direct client care.
- Will assess clients withdrawal symptoms per COWS and CIWA as dictated by protocols.
- Nurse will monitor efficacy of medications administered per Standing Orders and report concerns to appropriate medical providers.
- Administer prescribed, routine and PRN medications as scheduled no more than 1 hour before or 1 hour after of prescribed time.
- Will complete client assessments every Q shift X 3 days and document any changes in client's condition.
- Ensures that Physicians' or APRNs orders are evaluated with 2 hours.
2. Adminstrative
- Completes all required clinical documentation in accordance with Center Policy and Procedure and funding source guidelines.
- Review with client ASAM placement criteria and obtain signatures daily.
- Perform TB testing on each client upon admissions and document results 48-72 hours later.
- Will perform charting 3 X per 8-hour shift and 4 X per 12 hour and PRN.
- Will attend at least 95% of all scheduled nursing and all-staff meetings.
- Completes all required training within the appropriate time frames, i.e. Relias, HWC, CPR, etc.
3. Team Atmosphere
- Functions as a member of a multi-displinary team.
- Attends shift reports each shift with IC/OG nursing staff without tardiness.
- Participates in an on-call rotation as scheduled and returns calls within 15 minutes.
- Provides feedback regarding client's medical needs/assessments with provider, therapist and nursing staff.
- Maintains a positive and professional working relationships with all staff members and supervisors.
- Perform other duties as assigned by supervisor and other leadership positions within the program.
- Works independently while understanding the necessity for communicating and coordinating work efforts with other employees and organizations.

COMPENSATION:
- Starting salary for this position is approximately $22.99/hr based on relevant experience and education.
Schedule:
- This is a full-time position operating 24 hours a day, 365 days a year.
- This Regular full-time position requires 40 hour per week with understanding that the schedule is subject to change to meet the needs of the program.
- Must be flexible with work scheduling and participates in On-Call rotation as scheduled.
Travel:
- This position does not require any day trips or overnight.
Equipment/Technical Competency:
- Able to utilize computer skills, use of BAC machine, pulse oximeter machine.
- Able to read Urine Drug Screens with accuracy.
- Monitor CT's BGL using Glucometer, ability to utilize the Aneroid Sphygmomanometer and stethoscope, must be able to monitor vital signs and record in the electronic medical files.
Equipment/Technology:
- Able to chart accurate nursing assessment, will assess new client and integrate note in to a cohesive comprehensive assessment note in the Electronic Medical Records (EMR).
QUALIFICATIONS - Centerpointe LPN
Experience / Knowledge:
- Possess a working knowledge of basic nursing techniques.
- Demonstrates ethical and appropriate behavior for a clinical environment to include, but not limited to maintenance of confidentiality of patient information.
- The ability to retrieve, communicate and present data and information both verbally and in writing required.
- The ability to understand instructions.
- Possess medical terminology, charting and knowledge of the nursing process.
- Must be flexible with work scheduling.
- Participates in On-Call rotation as scheduled.
Education / License:
- Graduate of an accredited licensed Practical Nurse Program and possess a current nursing license.
Physical/Emotional/Social - Skills/Abilities:
- Nurse must complete HWC and CPR amongst continuing education modules as assigned.
- Able to lift up to 50 Ibs.
- The individual must also be able to maneuver throughout halls, client's rooms and surrounding facility in response to emergencies.
- A significant amount of writing and keyboarding is required.
