Summary of position:

The Clinician provides weekly individual and family psychotherapy to adults with mental health challenges. This position serves clients within both the First Episode Psychosis - OnTrack TN program (ages 15-30) and the Hamilton Adult IST therapist, delivering developmentally appropriate, evidence-based interventions to promote recovery, resilience, and family engagement.

JOB DUTIES/RESPONSIBILITIES

Job Responsibilities: Hamilton Adult IST Clinician

1. Works in a multi-disciplinary team approach to meet the needs of consumers.

    • Attends supervision on a bi-weekly basis without tardiness.
    • Attends administrative team meeting on a monthly basis without tardiness.
    • Responds to all flags, emails, and voicemails within 2 business days.

    2. Complete all documentation in compliance with CARF and SSOC standards.

    • Maintains a 90% benchmark for Crisis Plan completion.
    • Submits all progress notes within 2 - 4 days of the occurrence.
    • Completes treatment plan during first 30 days of treatment.

    3. Monitoring of treatment effectiveness and compliance, including assistance with medication.

    • 15% chart sample evidences in notes that therapist has addressed medication compliance with the client.
    • 15% of chart sample indicates in notes that client's goals are consistent with assessment.
    • 15% of chart sample indicates in notes that services are consistent with goals.
    • 15% of chart sample indicates in notes that clients are involved in their treatment.
    • 15% of chart sample indicates in notes that participants had positive outcomes.
    • 15% of chart sample indicates in notes that services related to goals/objectives.

    4. Provide linkage to other community supports, agencies, or services that are appropriate for client.

    5. Completion of expected productivity as outlined at the beginning of each month.

    Job Responsibilities: Hamilton County OnTrack Master's Level Clinician

    1. Provides individual/family psychotherapy for youth and young adults age 15-30.

    • Ensures therapy contacts are completed each month, according to program standards.
    • Attends and is timely to weekly multi-disciplinary team meetings
    • Attends weekly staff meetings with team.
    • Ensures updated treatment plans, consumer strength assessments, crisis plans, consent to contact, releases, ROI, MIRECC and updated admissions signature page every 6 months as evidence by quarterly quality record reviews for OnTrack TN clients
    • Routes all treatment plans to licensed supervisor

    2. Provides diagnostic Psycho-Social Assessments    

    • Clinician completes intakes and assessments within 5 business days of contact. 
    • Assessments will be completed in coordination with Assessment Team and Client Benefits as evidenced by communicating any issues concerning clients or assessment.

    3. Documentation requirements

    • Ensures all family and client participation in treatment planning as evidence by signatures on tx plans.
    • Communicates to supervisor if referrals become low in weekly supervisions.
    • Meet weekly with direct supervisor (OnTrack team lead) to discuss clinical and administrative concerns/ changes
    • Create and implement treatment interventions that are reflected in progress note documentation based on the treatment plan goals
    • Respond to all flags, emails, and voicemails within 2 business days

    4. Complete all required clinical documentation in accordance with Center Policy and Procedure and funding source guidelines.

      • Update treatment plans, crisis plans, DLA-20 and updated admissions signature page every 6 months
      • Complete and submit productivity summaries to supervisor by the 3rd business day of the month.
      • Create daily progress notes and sign in EMR within one week of service provided

      COMPENSATION:

      • Starting salary for this position is approximately $49,647/year based on relevant experience and education.

      Schedule:

      • Primary working hours are Monday through Friday 8am-5pm.

      Equipment/Technical Competency:

      • Must have basic competency in office applications such as Word and Excel.
      • Position requires regular use of a computer, and appropriate phone etiquette.
      • Position must be able to complete electronic documentation.

      QUALIFICATIONS - Hamilton Co. Adult IST & OnTrack MA Clinician

      Experience / Knowledge:

      • One year experience in the social work field or a behavioral health setting preferred.
      • Knowledge of etiology and treatment of mental illness, physical and sexual abuse, suicide, human development, and cultural diversity.

      Education / License:

      • Master's Degree in Human Services or related field with course work or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and developmental disabilities.
      • Degree must be license eligible

      Physical

      • Exposure to biological hazards. 
      • Hearing of normal and soft tones. 
      • Close eye work. 
      • Valid driver's license- "F" endorsement required for the transportation of clients. 
      • Lifting up to 50 lbs. Pushing/pulling up to 150 lbs. 
      • Frequent sitting, standing, walking, bending, stooping, and reaching. 
      • Required to be certified in, and effectively implement, verbal and physical de-escalation techniques that include a wide range of bodily movements; including but not limited to grasping, holding another person, going down on knees, running, and walking. Isolation and physical restraint is not used within clinician's HRMC programs.
      • An essential job function is a reliable, predictable 40 hour per week job, on site and in the community, with regular attendance.
      • Ability to effectively and ethically provide clinical services to children, young adults and families. 
      • Ability to present professionally and work within a team format to plan, implement, and evaluate successful interventions. 
      • Ability to effectively run and process groups. 
      • Ability to work within a team format to meet positive goals for children and adults and interface with other agencies involved in the ecology of the individual. 
      • Must be capable of assisting in utilizing nonviolent methods of crisis intervention including therapeutic holding. 
      • Ability to exercise sound judgment and effective decision-making
      • Ability to set and demonstrate appropriate boundaries, be an empathic listener, flexibility, willingness, and adaptability to working with diverse populations. 
      • Must also have the ability to communicate effectively and possess good time management and organizational skills.
