The HealthLink Team Leader

Summary of position:

  • Responsible for the direct supervision and oversight of a Health Link Team to ensure that the team maintains compliance with all standards and meets or exceeds all internal/external expectations.
  • Oversight duties include ensuring that high quality clinical care is provided, monitoring program productivity, managing caseloads, managing Health Link attestations/attributions/enrollments, monitoring clinical documentation, monitoring progress toward quality/efficiency metrics, conducting a variety of required reports, and directly managing front-line staff.
  • Provides coverage when needed to ensure all client needs and productivity targets are achieved.
  • This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment.

JOB DUTIES/RESPONSIBILITIES

1. Ensures that the Health Link program achieves its assigned productivity targets on a monthly basis.

  • Ensures that all clients assigned to the team receive a minimum of one face-to-face encounter per month.
  • Ensures that each Care Coordinator holds a caseload of a minimum of 85 clients, and contact each client on caseload at least 1x per month.
  • Maintains own case load of a minimum of 20 clients, and submits a total of 20 contacts per month.
  • Ensures that all clients assigned to the team have an up to date Care Plan/Crisis
    Plan/DLA-20
  • Ensures each supervised Care Coordinator completes/submits all documentation within 2 days.
  • Reviews all documentation for accuracy and completeness- meets all funder and CARF guidelines.

2. Administrative/Reporting

  • Takes a lead role in hiring and retaining staff; maintains appropriate level of staffing.
  • Fills staff vacancies within 2 weeks of previous employee's termination.
  • Trains staff regarding program P&P and monthly productivity goals within one week of completing orientation.
  • Ensures each supervised Care Coordinator attends treatment team 3x a month and individual supervision weekly.
  • Ensures each supervised Care Coordinator completes required CARF modules
    annually.
  • Maintain an up-to-date excel sheet to identify the currently enrolled clients in Health Link and the date of first contact each month
  • Review bimonthly and confirm each Health Link client's tenncare enrollment and potential renewal dates.
  • Submits team productivity report by the 5th of each month.
  • Prepares QA/QI and QRR reports for each program quarterly.

3. Meets Program Clinical Needs.

  • Works in a multi-disciplinary team to meet the needs of the SPMI population and
    oversees clinical standards of the program.
  • Handles client inquiries and addresses issues as appropriate.
  • Responds to all flags, emails, and voicemail within 2 business days.
  • Completes all chart documentation within 2 business days.
  • Ensures that each Care Coordinator maintains a minimum caseload of 85 clients.
  • Ensures each care coordinator has adequate referrals, and referrals are assigned as needed and appropriate to the team.

4. Promotes and Maintains Positive Teamwork

  • Organizes and develops team cohesion during staff meetings.
  • Meets with staff on a consistent basis to address identified concerns.
  • Demonstrates consideration and concern for fellow workers and promotes harmonious relationships and attitudes.

5. Networking

  • Maintains positive relationships with internal departments to meet program needs and maximize growth.
  • Develops a positive and working relationship with outside agencies to meet program needs.
  • Gives and receives appropriate feedback.
  • Communicates directly with a problem-solving approach.

COMPENSATION:

  • Starting salary for this position is approximately $20.68/hour based on relevant experience and education.

    Schedule:

    • Primary working hours are Monday through Friday 8am-5pm.

      Travel:

      • Potential travel required to family homes to provide services or meet families out in the community

        Equipment/Technical Competency:

        • Must have basic competency in office applications such as Word and Excel.
        • Position requires regular use of a computer, and appropriate phone etiquette.

        QUALIFICATIONS - HealthLink Team Leader

        Experience / Knowledge:

        • One year experience in the social work field or a behavioral health setting required. One year experience in a supervisory capacity preferred.

        Education / License:

        • Must have at least a Bachelor's degree in a health-related field of counseling, psychology, social work, sociology, or a behavioral science field with course work or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and intellectual disabilities.
        • Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, or education, must have fifteen college-level semester hours of coursework in behavioral health and at least one year of work experience in the behavioral health setting.

        Physical/Emotional/Social - Skills/Abilities:

        • Exposure to biological hazards.
        • Hearing of normal and soft tones.
        • Close eye work.
        • Valid driver's license. This position is required to obtain an F Endorsement on their driver's license.
        • Lifting up to 50 lbs. Pushing/pulling up to 150 lbs.
        • Frequent sitting, standing, walking, bending, stooping, and reaching.
        • Required to be certified in, and effectively implement, verbal and physical de-escalation techniques that include a wide range of bodily movements; including but not limited to, grasping, holding another person, going down on knees, running, and walking.
