Qualifications

Education/License: Bachelor's degree in Human Services or related field with coursework or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, and intellectual disabilities.  Demonstrated knowledge and competency in mental health service provision.

Experience/Knowledge:  Must have course work and or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and intellectual disabilities. One year experience preferred. Experience working with children is preferred although applicants may have other skills and experiences that could accommodate this position.

Physical/Emotional/Social-Skills/Abilities:  Ability to effectively and ethically counsel children and families.  Ability to present professionally and work within a team format to plan, implement, and evaluate successful interventions.  Ability to effectively run and process groups with children and families.  Ability to work within a team format to meet positive goals for children and interface with other agencies involved in the ecology of the family.  Must have mental ability to exercise sound judgment under pressure.  Ability to exercise effective decision-making, ability to set and demonstrate appropriate boundaries, ability to be an empathic listener, flexibility, willingness, and adaptability to working with diverse populations.  Must also have the ability to communicate effectively and possess good time management and organizational skills. CPR and First Aid Certification will be required. This position requires training and certification in and the ability to adequately implement a variety of verbal de-escalation methods and personal/client safety techniques.

Physical de-escalation techniques will only be implemented at McNabb Center facilities, and not in community settings. Hearing of normal and soft tones.  Close eye work. Valid driver's license and F endorsement. Lifting up to 50lbs. Frequent sitting, standing, walking, bending, stooping, and reaching.

Equipment/Technical Competency:  Must have basic computer skills in Microsoft Office, e-mail, and internet use.

COMPENSATION:

  • Starting salary for this position is approximately $18.42474/hour based on relevant experience and education.

Essential Functions

Schedule:  An essential job function is a reliable, predictable 40 hour per week job, on site and in the schools and/or community, with regular attendance.  This position provides services Monday-Friday during regular business, but can be adapted to client, family, and school system needs.

Travel:  This position requires utilizing a personal dependable vehicle to conduct Center business and maintaining a dependable vehicle and certified driver status is a condition of employment. "F" endorsement is required for occasional transportation of clients as needed. Position will oversee and provide support to all TDMHSAS Region II counties in East Tennessee.

Equipment/Technology: Must have basic computer skills in Microsoft Office, e-mail, and internet use.  Ability to utilize electronic medical records.

Duties:

Maintains ongoing communication with assigned schools

Completes case management for students identified by schools

Provides advocacy, linkage, and referral services as needed

Participates in IEP and other school related meetings, as applicable.

Provides education and training to schools and those served

Maintains appropriate chart records and completes all documentation in a timely manner

Upholds center policy and procedures, and CARF standards

Job Summary

Summary of position: Serves as a case manager for the regional school based crisis response team. Counties served include Knox, Scott, Morgan, Campbell, Claiborne, Anderson, Union, Grainger, Roane, Loudon, Hamblen, Jefferson, Cocke, Sevier, Blount, and Monroe. The program will partner with schools to provide trauma-informed consultation, education, training, crisis support, screening, assessment, and referral/linkage. This position will also support and train staff to offer debriefing to school when suicides or community violence occurs. Staff provides case management services to children ages 4-21 with a mental health diagnosis, as needed. Staff provides crisis intervention and emergency services, as needed. Interfaces with the school system and other agencies to increase cooperation and consistency between home, school, and community. Utilizes center wide and community services as needed for client benefit.  Embraces the key values of case management: empowerment, normalization, rehabilitation, and continuity of care.

Summary of role of team: The school based team serves as a treatment team to plan, implement, and evaluate successful interventions for children and families.  Works alongside schools to identify and collaborate about mental health services to selected schools while providing linkage and coordination to appropriate services and training, as requested.

Typical Working Conditions/Environment

Job duties are primarily performed in the community setting, but services are also provided in the school setting, homes, and office.  This position is year round, 40 hours per week and is not limited to the school schedule.

Quality Care

1.  Provides case management for children ages 4-21.

a. Case Manager is present and ready to begin session at the appointment time, every session.

b. Case Manager calls to reschedule any sessions that he/she will be unable to attend or that the consumer missed or canceled 100% of the time.

c. Case Manager communicates accurate and thorough information to outside agencies 100% of the time when clients need different or more appropriate care that HRMC cannot provide.

d. Case Manager contacts are completed each month, according to program standards.

e. Communicates to Team Leader in weekly supervision if referrals become low to ensure adequate and timely caseload assignments of clients on referral list

f. Meet weekly with direct supervisor to discuss clinical and administrative concerns/changes.

  1. Documentation requirements
    1. Ensures all family/client participation in tx planning as evidence by signatures on tx plans.
    2. Create and implement treatment interventions that are reflected in progress note documentation based on the treatment plan goals
    3. Respond to all flags and e-mails within 2 business days
    4. Respond to all voice mails within 30 minutes
    5. Attend monthly treatment teams and staffing meetings with Director/Program Coordinator/Team Leaders.
    6. Attend and participate in school based meetings with school personnel and community partners involved in the community schools.
  1. Complete all required clinical documentation in accordance with Center Policy and Procedure and funding source guidelines.
    1. Update treatment plans, crisis plans, consent to contact, DLA-20 and updated admissions signature page every 6 months
    2. Complete and submit productivity II summaries to Team Leader by the 3rd business day of the month.
    3. Complete initial treatment plans within 30 days of intake
    4. Create daily progress notes and sign in EMR within one week of service provided
    5. Submit all daily contact logs monthly.
    6. Submit hand generated productivity to Team Leader by the third working business day of the month.
    7. Creates and implements at least 1 measurable treatment intervention per treatment plan goal.
    1. Utilizes family strengths and empowers the family to implement effective interventions as evidenced by the client's progress toward goals.
    1. Makes referral to less intensive services upon anticipation of d/c.
  1. Leadership
    1. Develops and implements training topics/presentations that support enhancing trauma-informed environments in schools.
    2. Facilitate debriefing activities in schools when a crisis team is requested.
    3. Supports the crisis response team by implementing trauma-informed interventions and providing referrals for ongoing care that support student and family needs.
