SUMMARY - Health Link Care Coordinator
The ideal Health Link Care Coordinator serves as the primary care coordinator that provides case coordination using an integrative approach to a pre-determined number of children and/or adults. They function as a member of the treatment team to plan, implement, and evaluate successful interventions for children, adults, and/or families. They will interface with community agencies and referral sources to coordinate care, as well as to increase cooperation and consistency between home, school, and community. They will provide advocacy, linkage, and referral services as need, participate in direct supervision with the program supervisor and RN Coordinator as necessary, and participate in treatment team meetings with a RN Care Coordinator and/or psychiatrist/APN. They will also maintain appropriate chart records and complete documentation in a timely manner, provide medication monitoring, provide clients with therapeutic support and crisis intervention when needed, and uphold Center policy and procedures, as well as CARF standards.
The Health Link Care Coordinator must have the ability to effectively and ethically coordinate care for children, adults, and/or families. They must be able to present professionally and be able to work within a team format to plan, implement, and evaluate successful interventions. This person must be able to work within a team format to meet positive goals for the clients. They must have the mental ability to exercise sound judgment under pressure, the ability to demonstrate effective decision-making skills, the ability to set and then demonstrate appropriate boundaries, the ability to be an empathetic listener, the ability to be flexible, demonstrate willingness, and be adaptable when working with diverse populations. They must also be able to communicate effectively and possess strong time management and organizational skills. They must embrace the key values of case management: empowerment, normalization, rehabilitation, and continuity of care.
Compensation: Salary for this position starts at approximately $15.78 based on relevant experience and education.
Schedule: Work schedule is typically Monday-Friday, 8am-5pm in the office/community. Flexible hours may be required dependent on client need
Travel: This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment. This position also requires that the employee has an F Endorsement on his/her Driver License to be available to transport individuals as needed.
