the McNabb Center Team as a Case Manager for the On Track TN program in
Hamilton County, today!
The On Track
Case Manager will coordinate
outreach, recruitment, case management, education, and recovery coach duties
with clients enrolled in the program.
- Responsible for all outreach and
recruitment duties, initial referral screenings, and interdisciplinary
collaboration with the OnTrack team.
- She/he will help clients clarify goals,
cope with stressful situations, interact more effectively with other people,
and generally to overcome barriers to client's recovery.
- Provides information
and referral, agency linkage, advocacy, home visits and follow-up services as
defined by the Case Management Service Plan.
- Provides clinically appropriate
transition to clients transitioning out of the OnTrack TN program.
- Provides
crisis intervention and emergency services as needed.
- Utilizes center wide
and community services as needed for client benefit.
- Embraces the key
values of case management: empowerment, normalization, rehabilitation, and
continuity of care.
COMPENSATION: Starting
salary for this position is approximately $16.86 /hr based on relevant
experience and education.
Schedule:
- Flexibility
is required to be available as a member of the
OnTrack Team to case management clients 24 hours per day, seven days per week
by means of an on-call cell phone and provides direct coverage Monday thru
Friday.
Travel:
- F
endorsement is required.
Equipment/Technology:
- Use of
a Center issued Cell Phone and Laptop are required for email, scheduling, time
keeping, and documentation in the electronic medical record.
QUALIFICATIONS - On Track
Case Manager
Education:
- A Bachelor's degree in a health related field of counseling, psychology,
or social work or must be licensed as a nurse (LPN or RN). LPNs must have
fifteen (15) semester hours of behavioral health and one (1) years of
behavioral health care experience.
- Candidates with a Bachelor's degree in
rehabilitation, occupational therapy, criminal justice, and education must have
fifteen (15) college-level semester hours of coursework in behavioral health
and at least one (1) years of work experience in the behavioral health setting
working with children.
Experience / Knowledge:
- Must have course
work and or experience in the areas of cultural diversity, human development,
etiology and treatment of mental illness, alcohol and drug abuse, physical and
sexual abuse, suicide, and developmental / intellectual disability.
Physical:
- Minimal
exposure to biological hazards.
- Hearing of normal/soft tones and close eye work.
- Valid driver's license.
- Frequent sitting, standing, walking, bending, stooping, and reaching.
- CPR and First Aid
certification required (training provided).
- Required to be certified in and adequately implement verbal
de-escalation techniques.
- Applicants should be able to exercise sound judgement under pressure.
- Clinical staff may be required to get an F endorsement to transport
clients as necessary.
