The Hamilton County C/Y Homeless Outreach Case Manager provides homeless outreach services to children and families who are homeless or at risk of becoming homeless and case management to mentally ill/SED consumers at Helen Ross McNabb Center. Works as part of a multi-disciplinary team, providing intensive services to program consumers, primarily in community-based settings. 

COMPENSATION: Starting salary for this position is approximately $15.51/hr based on relevant experience and education.

QUALIFICATIONS - Hamilton County C/Y Homeless Outreach Case Manager: 

Education: 

  • Bachelor degree in behavioral science discipline from an accredited institution.  State licensure or certification when applicable. 

Experience / Knowledge

  • Ability to relate well to seriously mentally ill/SED individuals and their families.
  • Ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels, and economic circumstances with respect and dignity. 
  • Ability to communicate effectively, use strong time management and organizational skills.
  • Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation. 

Physical/Emotional/Social - Skills/Abilities:

  • Exposure to a variety of environmental factors including temperature extremes, precipitation, dust, fumes, odors and vermin. 
  • Ability to physically assist consumers negotiating barriers to mobility. 
  • Ability to write or dictate progress notes, sit for long periods, walk, run, climb stairs, stoop and occasionally lift moderate to heavy loads of 75 lbs. or less.
  • CPR/First Aid Certification required
  • Certification in and adequate implementation of verbal de-escalation techniques and escapes that include a wide range of bodily movements including but not limited to grasping, holding another person, running and walking
  • Hearing of normal to soft tones, distinguish smells and temperature changes.
  • Ability to work under moderate to intense stress and relate well to mental health consumers/SED
  • Ability to quickly escape dangerous or potentially dangerous situations while working alone with mental health consumers in a field environment.
  • Ability to obtain an F-endorsement on driver's license in order to transport clients in a personal vehicle. Must maintain certified driver status.
  • Ability to operate multiple lines and operate cellular telephones, keyboard, computer, copy machine, and fax machine.
