The Ideal Health Link Care Coordinator in Hamblen County provides case coordination using an integrative approach to a predetermined number of children and/or adults.  Interfaces with agencies to increase cooperation and consistency between home, school, work, community, and behavioral and physical health providers.  Functions as a member of a treatment team to plan, implement, and evaluate successful interventions for children, adults, and/or families.  Embraces the key values of empowerment, normalization, rehabilitation, and continuity of care. The holder of this position will provide contacts with individuals that meet the requirements of the program.

  • · Provides advocacy, linkage, and referral services as needed
  • · Maintains appropriate chart records
  • · Participates in direct supervision with the program supervisor and RN Coordinator, as necessary.
  • · Interfaces with community agencies and referral sources to coordinate care.
  • · Provides medication monitoring
  • · Completes all documentation in a timely manner
  • · Participates in treatment team meetings with a RN Care Coordinator
  • · Provides therapeutic support and crisis intervention as needed
  • · Upholds center policy and procedures, and CARF standards
  • · Will have at least 50 Face To Face with client each month and the other contacts will be indirect with client, PCP, teacher, etc. to coordinate services for the client.

COMPENSATION: Starting salary for this position is approximately $15.78 /hr based on relevant experience and education.

Schedule: This position has a regular schedule of Monday - Friday 8a.m. until 5p.m. There is no on-call for this position and employee is off on Center designated holidays. In person daily attendance is essential for this position except in instances of approved time off.

Travel: This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment. An F Endorsement is required as staff members may be required to provide transportation in their personal vehicle. Staff members are reimbursed for use of their vehicle for this purpose. This position may be required to travel to out of state conferences/meetings as applicable.

Equipment/Technology: This position does require basic computer skills for timekeeping, scheduling, use of electronic medical records, and extensive experience with Microsoft Office Products (Word/Excel/Outlook).

QUALIFICATIONS - Health Link Care Coordinator

Education:

  • Bachelor's degree in a health-related field of counseling, psychology or social work.
  • Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen (15) college-level semester hours of course work in behavioral health and at least one (1) year of work experience in the behavioral health setting.

Experience / Knowledge:

  • Must have course work and or experience in the areas of cultural diversity, human development, etiology and treatment of medical disorders and mental illness, alcohol and drug abuse, physical and sexual abuse, and suicide.
  • Computer experience is helpful.

Physical:

  • Minimal exposure to biological hazards.
  • Frequent exposure to unpleasant odors.
  • Hearing of normal and soft tones.
  • Close eye work.
  • Valid driver's license.
  • Lifting up to 50 lbs.
  • Pushing/pulling up to 150 lbs.
  • Frequent sitting, standing, walking, bending, stooping, and reaching.
  • CPR and First Aid certification required.
  • Required to be certified in and adequately implement verbal de-escalation techniques
  • Physical de-escalation techniques will only be implemented at McNabb Center facilities and not in community settings.
  • Applicants should possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels, and economic circumstances with respect and dignity. 
  • Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation. 
  • Clinical staff members must possess reliable transportation, the ability to acquire an F endorsement on driver's license, and transport clients.
