The Ideal Outreach Counselor provides identification, information and referral, and linkages to run away and homeless youth.  Meets with these youth and develops a trusting, therapeutic relationship with them while making them aware of available services.  Coordinates with other community partners to increase community awareness of available services to target population.  Coordinates with McNabb Center's marketing department to create and disseminate materials to target population.

COMPENSATION: Starting salary for this position is approximately $15.13 /hr based on relevant experience and education.

Travel: This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment. An F Endorsement is required as staff members may be required to provide transportation in their personal vehicle. Staff members are reimbursed for use of their vehicle for this purpose. This position may be required to travel to out of state conferences/meetings as applicable.

Equipment/Technology: This position does require computer skills for timekeeping, scheduling, use of electronic medical records entry, and extensive experience with Microsoft Office Products (Word/Excel/Outlook).

QUALIFICATIONS - Health Link RN Care Coordinator

Education:

  • Bachelor's degree, or commensurate experience, in social services, mental health, or youth-services field and experience with youth and knowledge of victims' rights, criminal justice system, and community resources is required.

Experience / Knowledge:

  • At least 1 year of experience working with at-risk youth is preferred.

Physical:

  • Minimal exposure to biological hazards.
  • Frequent exposure to unpleasant odors.
  • Hearing of normal and soft tones.
  • Close eye work.
  • Valid driver's license.
  • Lifting up to 50 lbs.
  • Pushing/pulling up to 150 lbs.
  • Frequent sitting, standing, walking, bending, stooping, and reaching.
  • CPR and First Aid certification required.
  • Required to be certified in and adequately implement verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, running, and walking.
  • Physical de-escalation techniques will only be implemented at McNabb Center facilities and not in community settings.
  • Applicants should possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels, and economic circumstances with respect and dignity. 
  • Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation. 
  • Clinical staff members must possess reliable transportation, the ability to acquire an F endorsement on driver's license, and transport clients.
