SOR Pathfinder (case manager)

The SOR Pathfinder:

  • serves as a member of the multi-disciplinary team providing direct engagement, assessment, referral and on-going follow up with clients. 
  • is able to complete assessments and other required documents (C-SSRS, GPRAs, etc.), track referrals, and complete updates.
  • maintains positive connections to internal and external stakeholders and referral sources. 

COMPENSATION: Starting salary for this position is approximately $16.86 /hr based on relevant experience and education.

Schedule: This position is expected to work 40 hours in a week. Typical hours are 8 am-5 pm, with flexibility for work outside of those hours to meet the needs of the program. In-person daily attendance essential for this position.

Travel: Must have an F-endorsement and be able to legally operate personal vehicle during the performance of job duties. This position requires travel the counties served in this program during the day as needed. This position requires transportation of clients in personal vehicle.

Equipment/Technology: Basic computer skills are required for time keeping, email, scheduling, spreadsheets (Excel), word processing (Word), and documenting in the electronic medical record.

QUALIFICATIONS - SOR Pathfinder

Education: 

  • Must have a Bachelor's degree in Counseling, Addictions, Social Work or other related field with two years of experience working with the population. 
  • Should have broad knowledge and understanding of Motivational Interviewing, stages of change, harm reduction and best practices regarding substance use disorder treatment and recovery.

Experience / Knowledge

  • Must have experience and understanding in the areas of cultural diversity, human development, and treatment of substance use disorder, understanding of trauma-informed care, suicide prevention, crisis referral and extensive knowledge of community resources. 
  • Experience working with special populations including individuals with Co-occurring Disorders, substance use disorders and mental health disorders.

Physical:

  • Minimal exposure to biological hazards.
  • Frequent exposure to unpleasant odors.
  • Hearing of normal and soft tones.
  • Close eye work.
  • Valid driver's license.
  • Frequent sitting, standing, walking, bending, stooping, and reaching.
  • CPR and First Aid certification required.
  • Required to be certified in and adequately implement verbal de-escalation techniques.
  • Applicants should be able to exercise sound judgement under pressure.
  • Applicants should possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels, and economic circumstances with respect and dignity. 
  • Effective communication skills, strong time management, and organizational skills are necessary to manage day to day job responsibilities.
  • Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation. 
  • Clinical staff members must possess reliable transportation, the ability to acquire an F endorsement on driver's license, and transport clients.
