CSU Discharge Specialist

The CSU Discharge Specialist performs appropriate and specific discharge planning services for CSU clients. This function is exclusive to this position in order to ensure continuity of care and includes the following:

  • Meets with each client daily to assess needs and acts as a liaison with other providers to access services to reduce recidivism. 
  • Contact and follows up with each community mental health center or other providers to ensure that scheduled aftercare appointments are made.
  • Makes referrals to specialized treatment programs.
  • Works to connect any specific needs regarding housing, etc. 
  • May accompany client to aftercare appointments if warranted. 
  • May also provide the direct individuals and group treatment services within the CSU.

COMPENSATION: Starting salary for this position is approximately $16.09 /hr based on relevant experience and education.

Scheduling: Monday - Friday 8am - 5pm  Regular attendance is an essential job function, and some flexibility may be required to meet the needs of service recipients. 

Travel: This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment. F endorsement is required.

Equipment/Technology: Basic computer skills are required for email, time keeping, scheduling, and documentation in the electronic medical record.

QUALIFICATIONS - CSU Discharge Specialist

Education: 

  • A Bachelor's degree in a health related field of counseling, psychology, or social work or non-degree and three years experience working with individuals with mental illness or special populations with co-occurring and dual diagnoses preferred.

Experience / Knowledge

  • Experience in working with special populations including individuals with Severe and Persistent Mental Illness, Co-Occurring Disorders, and Dual Diagnosis Disorders preferred. 
  • Must have intricate knowledge of community resources and exceptional customer service qualities. 
  • Computer experience is helpful.  
  • Minimum of one year's experience working in a crisis setting preferred.

Physical:

  • Minimal exposure to biological hazards.
  • Hearing of normal/soft tones and close eye work.
  • Valid driver's license.
  • Frequent sitting, standing, walking, bending, stooping, and reaching.
  • CPR and First Aid certification required (training provided).
  • Required to be certified in and adequately implement verbal de-escalation techniques.
  • Applicants should be able to exercise sound judgement under pressure.
  • Clinical staff may be required to get an F endorsement to transport clients as necessary.
