SUMMARY - LPN
The LPN - Behavioral Health Urgent Care Center serves as a clinical member of the multi-disciplinary team, providing treatment, rehabilitation, and support for client population. The LPN must be adept at multi-tasking. They will provide direct medical care to clients, including clients in varying levels of crisis, and will frequently work with SPMI and clients with co-occurring issues. They will assist with crisis interventions and medical emergencies, obtain vital signs each shift or more frequently as indicated. They administer intramuscular injections, communicate with PCPs during intake and discharge phases, and liaison with other agencies as indicated.
The LPN is responsible for triaging referrals, completing intake/assessments on all clients admitted during their shift, obtaining urine drug screens and pregnancy tests on admissions, obtaining admission orders, dispensing routine and PRN meds, taking off orders from prescriber and updating MAR, completing electronic and paper charting in a timely manner and participating in treatment team meetings. Additionally, the LPN may take calls from recently discharged clients requesting bridging prescription, follow-up calls on weekends on discharged clients, obtain prior authorizations and coordinate with pharmacies. The LPN collaborates with Mobile Crisis assisting with BHUCC client's being referred to higher level of care, completing S-BARs. The LPN's schedule requires rotating weekends. Regular attendance is an essential job function.
This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment.
Compensation: Starting salary for this position is approximately $19.92 /hr based on relevant experience and education.
