At the McNabb Center, our mission is "Improving the lives of the people we serve." Do you see yourself as a part of this life-changing work? If so, we are currently seeking a qualified and motivated professional for the following position: Homeless Response Team Registered Nurse Case Manager.
Serves as a clinical member of a multi-disciplinary team providing treatment, rehabilitation, and support for individuals and families who are or are at risk of being homeless; Serves as a clinical specialist for medication management issues; Serves as a liaison between pharmacies and medical providers, including PCPs, specialists, hospitals and inpatient facilities, to ensure client access to all prescribed medications; Ensures client charts are reflective of prescribed medications; Administers injections to clients as directed.
Conducts assessments of client functioning; Provides side-by-side support based on client goals; Provides referral, linkage, and advocacy services; Conducts community/home visits and follow up services; Provides crisis intervention and emergency services. Embraces the key values of case management: empowerment, normalization, rehabilitation, and continuity of care.
Education/Knowledge: Must have licensure as a registered nurse or licensed practical nurse in the state of Tennessee. Must be a graduate of an accredited, LPN program. Must have knowledge of mental illness. Must have either a Bachelor's degree in nursing or an Associate's degree with two years clinical experience.
Must have knowledge of medical terminology, charting skills and familiarity with medical settings. Must have the ability to relate positively with and be emotionally supportive of clients in the SPMI population. Must have the ability to work as part of a multi-disciplinary team.
Must have at least one year of experience working with the SPMI population. Must have a strong commitment to the right and ability of each person with an SPMI to live and work in the community while having access to competent and appropriate support services. Must have a demonstrated ability to abide by professional/ethical code of conduct and to establish supportive and respectful relationships with clients.
Applicants should possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels, and economic circumstances with respect and dignity. Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation. Clinical staff members must possess reliable transportation, the ability to acquire an F endorsement on driver's license, and transport clients.
Physical: Exposure to biological hazards. Hearing of normal and soft tones as well as close eye work. Valid driver's license. Lifting up to 50 lbs. Pushing/pulling up to 150 lbs. Frequent sitting, standing, walking, bending, stooping, and reaching. To ensure the safety of the clients and staff, this position requires certification in and adequate implementation of verbal and physical de-escalation techniques (Handle with Care) that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.
