Compensation: Starting salary for this position is approximately $56,796 /yr based on relevant experience and education.

SUMMARY - PACT Nurse (RN) Case Manager

Join our clinical team providing treatment, rehabilitation, and support for clients with SPMI

  • Provide medication management, conduct assessments of client functioning, and provide side-by-side support based on client treatment goals
  • Provide referral, linkage, and advocacy services while meeting with client; Conduct home visits and follow up services as defined by the PACT model
  • Provide crisis intervention and emergency services
  • Embrace the key values of case management: empowerment, normalization, rehabilitation, and continuity of care. 
  • Requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment.


  • Education/Knowledge: Must have licensure as a registered nurse. Must have either a Bachelor's degree in nursing or an Associate's degree with two years clinical experience.
  • Experience/Skills: Must have at least one year of experience working with the SPMI population. Must have a strong commitment to the right and ability of each person with an SPMI to live and work in the community while having access to competent and appropriate support services. Must have a demonstrated ability to abide by professional/ethical code of conduct and to establish supportive and respectful relationships with clients.
  • Physical Environment: Exposure to biological hazards. Hearing of normal and soft tones as well as close eye work. Valid driver's license. Lifting up to 50 lbs. Pushing/pulling up to 150 lbs. Frequent sitting, standing, walking, bending, stooping, and reaching. To ensure the safety of the clients and staff, this position requires certification in and adequate implementation of verbal and physical de-escalation techniques (Handle with Care) that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.
