SUMMARY - Family Treatment Case Manager
The Family Treatment Case Manager will conduct a timely case needs assessment to determine family wraparound needs and coordinate, monitor, evaluate, and advocate for a package of appropriate services to meet the specific needs of the families served; Formulate, in partnership with the parents, children, and families, goal plans based on assessment and integrated with the family's clinical treatment plan; Develop community contacts and a thorough knowledge of community resources; Aid in orienting new clients to the program, assisting in clients' understanding of program rules and compliance; Interface with community employers in the development of supported employment placements; Assist families to gain specific skills to get employment in an integrated community setting and increase self-sufficiency and overall well-being, including enhancing parenting skills, skills to maintain sobriety, and relapse prevention skills; Address transportation needs, including referral and provision of transportation as needed to meet set goals and contractual obligations from funding sources; Work collaboratively with the Care Coordination team to ensure family's needs are met; Maintain professionally written documentation of all work with or on behalf of clients; Provide aftercare to clients housed off-site through regular home visits and telephone contact; Provide education groups to families living on-site; Provide shift coverage as needed for residential services; Participate as a Recovery Services team member providing programmatic input for Treatment Team decisions related to client needs and progress; Other duties assigned.
This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment.
REQUIREMENTS - Family Treatment Case Manager
Education/Knowledge: Bachelor's Degree in social services or a related human service field; Training in trauma informed care, co-occurring disorders, and child development preferred.
Experience: One year experience providing working with parents with co-occurring disorders and/or children affected by parental substance use preferred.
Applicants should possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels, and economic circumstances with respect and dignity. Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation. Clinical staff members must possess reliable transportation, the ability to acquire an F endorsement on driver's license, and transport clients.
Physical: Must be able to drive several hours per day; valid driver's license with "F" endorsement; frequent sitting, standing, walking. Must be able to work in residential program, including providing shift coverage for overnight and weekend shifts as needed. Hearing of normal and soft tones as well as close eye work. To ensure the safety of the clients and staff, this position requires certification in and adequate implementation of verbal and physical de-escalation techniques (Handle with Care) that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.
Compensation: Starting salary for this position is approximately $15.13 /hr based on relevant experience and education.
