Discharge Specialist (Crisis Stabilization Unit) Hamblen

If so, we are currently seeking a qualified and motivated professional for the following position: Crisis Stabilization Unit Discharge Specialist - Hamblen. 


The Discharge Specialist schedules appropriate and specific discharge planning services for CSU clients. This function is exclusive to this position in order to ensure continuity of care.  Meets with each client daily to assess needs and acts as a liaison with other providers to access services to reduce recidivism. Contact and follows up with each  community mental health center or other providers to ensure that scheduled aftercare appointments are made, makes referrals to specialized treatment programs, and also works to connect any specific needs regarding housing, etc. May also accompany client to aftercare appointments if warranted. Works closely with the CSU Team Leader and CSU resident counselors to maintain continuity. Serves as a clinical member of the multi-disciplinary team, providing treatment, rehabilitation, and support for client population.  May also provide direct treatment to individuals and group treatment services within the CSU.

Regular attendance is an essential job function Due to CSU and WIC being 24 hours/7 days per week programming, the following expectations are applied to all staff working in these programs: 

·  All staff will be present and on time for shift in order to relieve previous shift.

·  All staff will stay on shift until relief coverage arrive. 

·  All staff have been briefed on the nature of the programming and understand that shifts must be covered in order to keep our staff and clients safe.

All staff recognize that if bad weather is predicted be prepared to make it in prior to poor travel conditions and to stay until relieve staff can make it in.

REQUIREMENTS - Discharge Specialist

Education/Knowledge: A Bachelor's degree in a health related field of counseling, psychology, or social work or non-degree and three years experience working with individuals with mental illness or special populations with co-occurring and dual diagnoses preferred. Must obtain F endorsement.

Experience: Must have course work and or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and crisis intervention.  Must have experience in working with special populations including individuals with Severe and Persistent Mental Illnesses and Co-Occurring Disorders.  Experience working in a crisis setting preferred. Experience with scheduling appointments in a healthcare setting is helpful.

Applicants should possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels, and economic circumstances with respect and dignity.  Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation.  Clinical staff members must possess reliable transportation, the ability to acquire an F endorsement on driver's license, and transport clients.

Physical: Exposure to biological hazards. Hearing of normal and soft tones as well as close eye work. Valid driver's license. Lifting up to 50 lbs. Pushing/pulling up to 150 lbs. Frequent sitting, standing, walking, bending, stooping, and reaching. To ensure the safety of the clients and staff, this position requires certification in and adequate implementation of verbal and physical de-escalation techniques (Handle with Care) that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.
