FACT Case Manager
The FACT
Case Manager is responsible
for:
- Providing
information and referral, agency linkage, advocacy, community visits, and
assisting with obtaining permanent stable housing, and follow-up services as
defined by the Case Management Service Plan to individuals referred through the
criminal justice system and having a mental health, substance use or
co-occurring disorder.
- Provides crisis intervention and emergency services, as
well as utilizes center-wide and community resources as needed for client
benefit.
- Provide direct service delivery in the community within flexible
hours.
COMPENSATION: Starting
salary for this position is approximately $16.86 /hr based on relevant
experience and education.
Schedule:
- Non-traditional
hours. Flexibility is required.
Travel:
- This position requires utilizing a personal
dependable vehicle to conduct Center business.
- Maintaining a dependable vehicle
and certified driver status is a condition of employment.
- This position also
requires that the employee has an F endorsement on his/her Driver License to be
available to transport individuals as needed.
QUALIFICATIONS - FACT Case Manager
Education:
- Bachelor's degree in human services or related field with course work or
experience in the areas of cultural diversity, human development, etiology and
treatment of mental illness, alcohol and drug abuse, physical and sexual abuse,
suicide, and developmental / intellectual disability.
Experience / Knowledge:
- One year
experience in the social work and addictions field and/or behavioral health and
criminal justice setting preferred.
- Must be able to perform job
responsibilities on an independent level within non-traditional hours and
interface professionally with law enforcement and other identified agencies.
Physical:
- Minimal
exposure to biological hazards.
- Hearing of normal/soft tones and close eye work.
- Valid driver's license.
- Frequent sitting, standing, walking, bending, stooping, and reaching.
- CPR and First Aid
certification required (training provided).
- Required to be certified in and adequately implement verbal
de-escalation techniques.
- Applicants should be able to exercise sound judgement under pressure.
- Clinical staff may be required to get an F endorsement to transport
clients as necessary.
